What insurances do you accept?

We do not accept insurance.  We have chosen to be an out-of-network provider in order to be able to better serve our patients and provide them with the one-on-one care they deserve.  We recommend calling your insurance provider to find out about your out-of-network physical therapy benefits to determine what steps to take for reimbursement if you are eligible for out-of-network benefits.  If needed, we will provide step-by-step instructions for submitting an out-of-network claim for your insurance.  If we don't already have an instruction form for your insurance, we will help you find the appropriate forms.

 

What should I bring to my first appointment?

You should bring your insurance card, new patient forms, referral or prescription from your doctor (if given), any other medical reports given to you by your doctor and method of payment (we accept credit cards, debit cards or personal checks).

What should I wear to my first appointment?

Please wear loose fitting clothing that allows us to access the injured body part and allows you to move freely.  You will be able to change at the office if needed.

What forms of payment do you accept?

We accept all major credit cards, debit cards, checks, cash, and cards associated with HSA and FSA accounts.

What conditions do you treat?

We treat a wide variety of orthopedic conditions, as well as some neurological conditions.  We recommend contacting us so we can discuss your case and determine if we are the right clinic for you.  If not, we will help you find the right therapist in the area for your individual situation.
